Passport Application Requirements

IMPORTANT:

• Please be advised that during the months of December and January you must contact the Embassy before travelling to check if the Consular Officers are available on the prescribed days.
• Please bring EXACT CASH when making payments to the embassy.
• Applications will be received during consular hours before 12:30pm.

The applicant has to visit the Embassy in person for fingerprinting and authentication of documents. The appropriate forms will be issued to you upon your arrival.

No appointment required.

Application Process

The following original documents, plus clear photocopies, must be produced when applying for a passport:

1. Passport (current or expired)

2. National Identity Card (plastic, metal, or green paper NID with attached photo and printed NID number) – order of names should be as they appear on the NID and Birth Certificate; spellings have to be consistent throughout; if names are not in the order as they appear on the NID and Birth Certificate, applicant has to regularize by either changing the order of names on the NID and Birth Certificate (NID cards are only issued in Zimbabwe) – note: NID is required for all applicants 16 yrs and older.

3. Birth Certificate (long version) – if you have a short birth certificate, you may obtain a long one by authorizing your relative in writing to obtain one for you through the Registrar of Births in Harare (send a copy of your short birth certificate)

4. Two (2) passport colour photos (size 3,5 x 4,5 cm) – photo requirements: white background, appearance – black/dark grey/navy blue shirt/top/dress (no spaghetti straps), no eyeglasses, no jewelry, natural hair tied back, face front with both ears visible.

5. Marriage Certificate/National Identity Card in married name/Divorce Order/Spouse’s Death Certificate (if applicable).

6. Adoption Order (if applicable).

7. Proof of Legal Guardianship (if applicable).

8. Police report (for lost passport)

The applicant has to visit the Embassy in person for fingerprinting and authentication of documents. The appropriate forms will be issued to you upon your arrival.

Fees

CDN $100 Passport form and processing.

CDN $100 Authentication of drivers license.

If a child under 18 yrs is applying for a passport, Section 5 of the form must be completed by the parent – a legal guardian can also sign Section 5 if he/she can produce documentary proof of guardianship – a clear copy of the National ID or valid passport of the parent/legal guardian is required.

Once your forms have been processed and documents authenticated at the Embassy, everything will be returned to you. You must send your forms and other documentation to a relative in Zimbabwe preferably with the same last name, together with your current/expired passport (original). The relative will submit the forms to the passport office. Charges are as follows:

When children turn 16 years of age, they should obtain National Identity Cards. These can only be issued by the Registrar General's Office in Zimbabwe and not at the embassy.

Your relative should be able to provide you with a tracking number given by DHL or Federal Express. Around the same time you expect your documents to arrive at the Embassy, send us a self-addressed, courier envelope, together with a cover note indicating that it is for your passports from Zimbabwe. We recommend a method with a tracking number, such as Express Mail from any Canada Post Office. Once we receive your passports and envelope, we will mail them to you.

Passport application forms expire 6 months from the date of completion. Ensure that your complete application package is received by the Passport Office in Harare before the forms expire.
